After a serious illness, such as childhood cancer, lifelong routine check-ups are vital for monitoring health and wellbeing. However, at Cincinnati Children's, not ever patient transitions to "survivorship care" after they are cured from their cancer.

This was a continuation of a project that the LiveWell Collaborative has been completing with the Cincinnati Children's Cancer and Blood Diseases Institute (CBDI). The project has lasted multiple semesters, and is attempting to help encourage every patient to transition to survivorship care.

In collaboration with the CBDI staff and Survivorship team, the LiveWell team consisted of Andrea Maravic, Fiat Tongrod, and Claire Minton. We were led by Diego Gomez, and advised by Todd Timney.

How might we redesign the survivorship program in order to retain more patients?

In order for patients and families to understand the importance of survivorship, they must be educated! Education about what survivorship is, why it matters, and what CCHMC has to offer specifically.
Mitigating the large and overwhelming barrier that the scheduling process creates; making the process simpler for all stakeholders: patients, scheduling staff, social workers, and more.
Post-cancer health is so much more than preventative care. A balanced health approach considers emotional, mental, and physical health - as well as various aspects of lifestyle and habit.
